How we use your information
The CCG is responsible for buying (also known as commissioning) health services from healthcare providers such as hospitals, for our local population.
We also monitor the performance and quality of these services. In general we only use data that has been anonymised or pseudonymised for these purposes.
There are certain circumstances in which the CCG will have access to identifiable data about patients, for example:
- when responding to concerns/complaints from patients about commissioned services
- when handling Individual funding requests
- when handling NHS Continuing Healthcare applications
- for the purposes of safeguarding adults or children
The CCG may also share de-identified statistical information with NHS and partner agencies for the purpose of improving local services, for example understanding how health conditions spread across our local area compared to other areas.

Accessing Records held by the CCG
If you wish to access any information the CCG may hold about you, please contact our Information Governance (IG) Manager to make a Subject Access Request (SAR):

Requests are handled in line with our Subject Access Requests (SAR) Procedures and you can use the Subject Access Request form to make your request if it’s helpful.

Summary Care Information
What is the Summary Care Record (SCR)?
A Summary Care Record is an electronic record which contains information about:
- the medicines you take
- allergies you suffer from
- any bad reactions to medicines you have had
Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.
What do I need to do to get a Summary Care Record?
Nothing. If you are registered with a GP, a record will be created for you.
What do I need to do if I don’t want a Summary Care Record?
If you don’t want a Summary Care Record, you need to opt out. You can opt out by filling in an opt out form at your GP Surgery. Your GP surgery will mark your record so that a Summary Care Record is not created for you. This can be done at any time and you can change your mind.

Help with Health Information Online
You can find lots of health information online, whether it is about eating healthily and looking after yourself, facts about health conditions, or information on health services in your area.
All our member GP practices now have a website. You can find links to all practice websites on the Our Member Practices page. At some GP Practices, you can also book an appointment or order a repeat prescription using the practice website.
GP Practices are also working to make your records available to you online - you can find more information about this on the NHS Choices website
